Report abuse or safety issue

ProForms is a form platform used by agencies and businesses. We do not allow forms used for phishing, malware distribution, credential theft, impersonation, spam, or illegal collection of sensitive data.

Send a report

Email security@proforms.io with the form URL and a short description of the issue. We review abuse reports promptly and may disable forms, domains, API keys, or accounts that violate our policies.

Helpful details to include:

  • The full ProForms URL or client page where the form appears
  • Why you believe it is phishing, malware, spam, or impersonation
  • Any screenshot or message that led you to the form

If you found a vulnerability in ProForms itself, use the same security inbox and include reproduction steps. For privacy questions, contact privacy@proforms.io.
